Only the information required to provide safe and effective
massage therapy is collected.We
ask for phone numbers and email addresses in order to notify of any
changes in appointment times and cancellations.
All files are kept in a secure location for a minimum of 10
years from the last appointment date, as required by the Regulated Health
Professions Act (RHPA).The files
are then shredded for disposal to maintain confidentiality of information.
Only RMTs at Galt Massage Therapy will access the information
in the file.If the client was
referred by/to another health care professional, we may provide treatment
related information to that person.Clients may request, in writing, that their information is not to
be disclosed to consulting healthcare providers.
We will collect, use and disclose information about clients for
the following purposes:
To provide healthcare
To establish and maintain communication
For teaching and demonstrating purposes on an anonymous basis
To complete and submit claims for third party adjudication and
payment
To comply with legal and regulatory requirements according to
the provisions of the RHPA and Massage Therapy Act
To comply generally with the law
Clients have the right to see and read their files at any time
upon providing reasonable notice.We will assist the client in understanding the file.
In case of a breach of privacy, we are obliged to notify the
client of the theft, loss or unauthorized access.
